How to File Compliant or Provide Feedback Procedure

Please let us know your comments, concerns or suggestions promptly.
Please follow the following steps if you have a specific issue:

Step One:

Please speak to the Charge Nurse in your home area first. If they do not have the answer, they will get it for you, or pass your comments along to the appropriate department head that will get back to you. If you do not receive an adequate reply, proceed to step two.
Alternatively, if your concern involves more than one area it is best to request a Care Conference. You can request this at any time by booking with your Charge Nurse or calling the nursing administrative assistant at ext. 227. A Care Conference involves having a meeting with various department managers.

Step Two:

Talk to the department head responsible for the area you wish to discuss. Simply email info@bethanylodge.org or call reception at (905) 477- 3838 ext. 289 and ask for the following people:

  • The Director of Care
  • The Assistant Director of Care
  • Environmental Manager
  • Dietary Manager
  • Life Enrichment Manager

Step Three:

After discussing it with the Department representatives, if your suggestion or concern remains unresolved contact the Administrator, Basil Tambakis at ext. 224.

Step Four:

If your concern still remains unresolved after speaking with the Administrator, please contact the President, Board of Directors, John Paul @ johnp@ahearn.com.

Step Five:

If you feel your concern has not been adequately addressed, please feel free to call the Long-Term Care Family Support and Action
